Job Summary:

Coordinates the interdisciplinary team in resident assessment and development of resident plan of care according to the Resident Assessment Instrument (RAI) and state and federal requirements.

Job Responsibilities:

• Assess residents in order to accurately complete the Minimum Data Set and Resident Assessment Protocol.Coordinate MDS and Care Plan Team
• Implement and monitor individual care plans for residents based on MDS Guidelines.
• Maintain a schedule of due dates for MDS and Care Plans including new admissions and those with significant changes.
• Submit the MDS to the appropriate agency.

Job Qualifications and Skills:

• Current registration as an LPN or RN
• Strong written and verbal communication skills
• Ability to prioritize and make sound nursing decisions within scope of practice
• Possess leadership skills
