We are seeking a full-time Registered Nurse (RN) with experience in Diabetes & Endocrinology to join our team. In this role, you will partner closely with our Diabetes & Lifestyle Medicine Physician, Clinical Pharmacist, Care Navigators, and other health professionals in a collaborative care model. You’ll rotate through four of our Health & Wellness Centers, serving retired San Antonio First Responders and their families. (Clinic sites include Westover Hills, Fair Oaks, Schertz, and Roosevelt. You will typically only work at one site per day.)

Provide, plan, and evaluate patient care in alignment with physician orders and organizational standards

Support endocrinology and diabetes care, including procedures, medication administration, phlebotomy, and lab work

Educate and empower patients through our wellness and nutritional coaching programs

Collaborate with physicians, pharmacists, and navigators to support coordinated, value-based care

Document patient encounters accurately in the EMR, supporting coding and claims processes as needed

Respond to medical emergencies, provide injections, and assist with follow-up care and referrals

Lead and support clinic operations as needed, serving as a resource and mentor to LVNs, MAs, and other staff

Act as a steward of Health by Design’s mission and culture

 

Registered Nurse (RN) license in Texas (required)

Basic Life Support (BLS) certification (required)

Associate’s Degree, Hospital Diploma, or Bachelor’s Degree in Nursing (preferred)

Minimum of two years direct patient care experience

Prior Endocrinology/Diabetes experience highly preferred
