Description

VNA Hospice of NWI is seeking: Clinical Liaison, RN Coverage area includes Porter, LaPorte, Lake, Jasper, Newton and Starke. Mileage paid if a fleet car is not available.

SUMMARY: The Clinical Liaison is responsible for acting as a partner between the hospital and the agency by ensuring collaboration of care, coordinating discharge planning, and maintaining excellent communication. They will assist in developing relationships with physicians and hospital staff, meet with patients and families to educate them on VNA services, communicate with intake on the next steps, and ensure patients get the right service at the right time. The Clinical Liaison provides nursing care to patients who require GIP level of care in the hospital setting. 

Requirements

QUALIFIED CANDIDATE WILL POSSESS: 

• Unrestricted RN license in the State of Indiana
• Diploma must be from an approved school of professional nursing
• Excellent communication and organizational skills
• Minimum experience of 2 years medical surgical, critical care or oncology nursing
• Hospice experience preferred
• Must maintain current CPR certification
• Must hold and maintain a valid Indiana driver’s license and automobile insurance
• TB status must be current
