We’re looking for a Clinical Navigator – Float RN to join our primary care team.  This position will rotate between our Westfield and Fredonia offices throughout the week, with plans to be fully based in Fredonia by Fall 2025. 


This role offers a balance of consistency and variety—you’ll work closely with a dedicated provider to room patients and support daily clinical operations, while also floating as needed to support non-licensed staff in the office.  You’ll be part of a collaborative team that values strong communication, individualized care, and mutual support. This is a great fit for an RN who thrives in a fast-paced outpatient setting, enjoys direct patient interaction, and appreciates a predictable weekday schedule Monday – Friday, with no nights or weekends. Our office is an approved site for the National Health Service Corps (NHSC) loan repayment program.

Location: In-Office - Westfield NY and Fredonia NY with plans to be fully based in Fredonia by Fall 2025

Travel Required: Varies 

Position Type: Full-time, Non-Exempt

Pay Information: $25.50 - $29.50 per hour


SUMMARY:

The Clinical Navigator – Float RN role collaborates with providers and the Clinical Navigator team to manage the patient’s needs based on their individual condition and goals while also providing direct patient care in-office using current standards for preventative measures.


ROLE AND RESPONSIBILITIES:

  • Obtain vital signs and perform medication reconciliation during rooming process in a timely and professional manner
  • Use Disease Management and Health Maintenance as age-based tool to assist with and create orders, administer vaccines, and other incentive measures
  • Perform various medical procedures and assist providers
  • Handles nurse schedule for non-provider office visits; EKGs, 2 step oxygen testing, breathing treatments, ear lavages, immunizations, blood pressure checks, injections, etc.
  • Medication administration
  • Diabetic education
  • Assess patient medical conditions over the phone and for walk-in patients
  • Phlebotomy
  • Wound changes/dressings
  • Collaboration with MAs for immunizations and injectable medications
  • Call center overflow- assessment/insight for incoming calls
  • Triage tanks- TRANN cares, Navigation overflow, STAT provider triages, prior auths
  • Communication with facilities for STAT results/scheduling tests
  • Any additional duties as assigned by management



Requirements

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

  • Current CPR Certification obtained within 30 days of hire required
  • At least two years of healthcare experience in a related setting required
  • Bilingual preferred
  • Current Registered Nurse licensure in New York State required

  • Knowledge of transitional care

PHYSICAL REQUIREMENTS/WORKING CONDITIONS

The physical requirements and working environment are typical of most medical office settings. Employees are frequently required to sit for long periods of time, stand, walk, balance, hear, talk, reach, and use their hands. Employees may occasionally be required to bend, stoop, kneel, crouch, climb (stepstool) and stretch. Physical effort requires some lifting usually not in excess of 15 pounds. The office work environment is indoors and free from dust, smoke, moisture, and excessive noise.
