What Will You Do?
As a Care Team Coordinator, you will provide essential administrative support to the clinical staff—including nurses, social workers, paramedics, and other care team members—while assisting Participants enrolled in the TANDEM365 program. You’ll also play a key role in outreach and referral conversion, helping to ensure that eligible individuals are successfully enrolled and supported.
Responsibilities
- Provide phone support to Participants and external agencies
- Assist with Participant care coordination and scheduling.
- Conduct outreach to referrals and support enrollment.
- Collect healthcare information from PCPs and partner agencies.
- Maintain accurate records in the EMR system.

Education and Experience
- Minimum of 2 years of experience as a Medical Assistant or other clinical role (e.g., EMT, LPN), or at least 5 years of experience in a healthcare setting.
- Experience with Microsoft Office software preferred.
- Customer service experience in a health care setting highly valued.
- Proof of a valid Michigan driver’s license, auto insurance and reliable transportation.
