Your Job:In this highly technical, fast-paced, and challenging position, you'll collaborate with multidisciplinary team members to provide the very best care for our patients. Through effective relationships, the ER Patient Navigator (Navigator) helps patients/family recognize patterns of exacerbation of chronic conditions/illnesses and resource use. Helps the patient define goals for health management and quality of life. Your Job Requirements: • Master of Science, Nursing required (or Bachelor of Science in Nursing with equivalent program experience will be considered with Executive Approval) or Masters in Social Work required • Bachelor of Social Work with equivalent program experience will be considered with Executive Approval • Minimum 5 years of health care experience • Knowledgeable of multiple case management applications • Proficient with clinical issues for populations served • Must have computer knowledge and competency with office suite (e.g. Excel, Word, PowerPoint) Your Job Responsibilities: • Communicate clearly and openly • Build relationships to promote a collaborative environment • Be accountable for your performance • Always look for ways to improve the patient experience • Take initiative for your professional growth • Be engaged and eager to build a winning teamMethodist Charlton Medical Center is a 314-bed, full-service, acute care teaching hospital that serves Cedar Hill, DeSoto, Duncanville, and Lancaster in southwestern Dallas County.
