Role Overview:
The Older Youth Case Manager will work within a specialized caseload of DCS youth during adolescence to support successful transition to adulthood and independence. They will participate in the coordination of independent living services as well as promoting those in their youth workload. The employee will conduct assessment and planning according to the individual needs of youth, attend court, maintain database documentation, and outreach to the local field office to ensure that all staff eligible for services in Independent Living are receiving them at the age junctures outlined in policy.

This position serves these West Central Indiana counties: Clay, Owen, Parke, Putnam, Vermillion, and Vigo. You may occasionally serve in Green, Monroe, and Sullivan counties as needed. Residency in the service area is preferred.
Salary:
The salary for this position traditionally starts at $47,320.00 but may be commensurate with education or work experience.
