DESCRIPTION: 

To coordinate referral of new patients to the Regional Cancer Program, through retrieval of appropriate tests and results, appropriate appointment booking and contact with the patient/designate.

                

Within this role the employee is accountable for contributing to the delivery of the Kingston General Hospital strategy.  As an employee, one must demonstrate an awareness of and be responsible for actively promoting and supporting patient and family centered engagement and care in all we do.

 

 

DUTIES AND RESPONSIBILITIES:

  • Maintains strict confidentiality of information handled on a day to day basis and ensures that only authorized personnel are allowed access to patient information
  • Determine urgency and coordinate appropriate physician based on service (ex; medical or radiation, surgical oncology, hematology, plastics and psycho-social oncology), site and the case distribution according to Program guidelines.  Follow up with oncologists, organize work within NPR office to ensure consistency of workload, liaisons with other departments to ensure continuity of care.
  • Telephone patient direct and carry out interview to:  Obtain and verify all demographic data, to obtain investigation reports information, provide directions to the Centre and parking information, and information about the consultation visit, to notify patients of support services and to respond to patient questions or redirect inquiries to appropriate physician. Upon verbal consent from the patient, send electronic referral package via email to the patient/designate.  Cancels and/or reschedules appointments as required.
  • Assess referral for completeness, coordinate and order diagnostic reports and films, arrange delivery of reports and films, request pathology slides, retrieves appropriate reports from PCS/externa sources, CNEO and order hospital charts for referral patients as necessary, maintain referral database, identify delays if appointments outside of CCO acceptable wait times.  Completes electronic referral package and sends to physician for approval.
  • Creates within or confirms with hospital/OPIS systems the patient’s demographic data and updates as necessary.   Creates the new patient record (electronic and hard copy) and places appropriate forms within the record.  Creates/activates visits in PCS, OPIS and ARIA.   
  • Participates in other duties as required for quality assurance (eg. Provide input on standard operating procedure development related to NPR, attend meetings as required)          

 

 

BASIC QUALIFICATIONS:

  • 2 year Office Administration Diploma or equivalent combination of education and experience.
  • Minimum 2 years related experience in a health care environment within the last 5 years.
  • 50 wpm on test with a minimum of 95 % accuracy.
  • Demonstrated knowledge of Medical Terminology with 85% on oncology medical terminology test.
  • Demonstrated ability to communicate effectively, courteously and professionally.
  • Demonstrated evidence of strong organizational skills
  • Ability to follow instructions accurately, to cope with frequent interruptions and to meet deadlines
  • Must work harmoniously with other staff in the department and throughout the hospital to provide rapid, efficient health care. 
  • Demonstrated ability to work in Microsoft Outlook, Excel, Adobe, and Microsoft Word.
  • Ability to work both independently and collaboratively as a team.
  • Proven ability to attend work regularly.
  • Satisfactory criminal reference check and vulnerable sector search required.
