PURPOSE STATEMENT   

Responsible for providing high-quality patient care in a fast-paced, specialized clinical setting. Assist and work closely with providers and staff to assess, plan, implement and evaluate patient care by following established standards and practices. Functions as primary liaison between patients, staff and providers.


ESSENTIAL FUNCTIONS  

  • Obtains and documents medical history, including chief complaint, risk factors, disease condition and medication reconciliation; Assesses and documents vital signs and pertinent physical exams. 
  • Provides education to patients and their families regarding health status, acute and chronic medical conditions, care plan and treatments, medication management and results from testing; Provides education for and obtains patient consent for care, as applicable. 
  • Provides care based on practice guidelines, standards of care, and federal/state laws and regulations; Executes prescribed treatments and interventions; Administers prescribed medications; Monitors patient progress and response to treatment. 
  • Collaborates with providers and multidisciplinary teams to develop and implement individualized care. 
  • Maintains and reviews patient records and other pertinent information; Documents patient assessments and interventions based on established guidelines, utilizing electronic health records (EHR) systems and/or approved medical record forms. 
  • Responds to incoming patient phone calls based on clinic guidelines; Effectively communicates with providers regarding any concerns or issues, including testing and procedure results. 
  • Serves as an advocate for patient concerns and care, including delegating as allowed under the Nurse Practice Act and consulting with providers and community resources to prevent/resolve issues and concerns. 
  • Participates in multidisciplinary teams to contribute and improve patient care processes and outcomes, tracks quality assurance data. 
  • Maintains patient confidentiality. 
  • Maintains regular and predictable attendance. 
  • Performs other essential duties as assigned.

Requirements

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: 

  • High school diploma or equivalent. 
  • Graduate of accredited school of professional nursing. 
  • Currently licensed to practice as a professional Registered Nurse in the state of Missouri or be eligible to practice as a graduate nurse in the State of Missouri. 
  • Current American Heart Association (AHA) BLS required.
  • Current American Heart Association (AHA) ACLS preferred. 
  • Minimum of one-year professional nursing experience, cardiology experience preferred. 
  • Must be self-motivated and have the ability to work within the policies, procedures and practices established by the hospital/clinic. 

PHYSICAL/MENTAL REQUIREMENTS: 

  • Must be able to frequently sit, stand and walk, sometimes on uneven surfaces, 8 to 10 hours a day.
  • Must be able to use standard office equipment, including a telephone and computer keyboard.
  • Must be able to use standard medical equipment or be trained to do so, including basic monitoring equipment and EKG machines.
  • Must be able to continuously work under expectations of near 100% accuracy and meet deadlines. 
  • Must be able to continuously utilize manual/bi-manual dexterity, near vision, speech, and hearing.
  • Must be able to utilize eye/hand coordination and color definition.
  • Must be able to regularly lift and/or carry up to 40 pounds and occasionally reach above shoulders.
