On-call Care Coordinator for Jordan & Mary Claire House
Department: Behavioral Health Services Location: Washington , DCSOME (So Others Might Eat) exists to help homeless and poor individuals and families in Washington, DC. Our more than 30 programs throughout the district provide comprehensive services that lead to mental and physical health, continued sobriety, job training, living-wage employment, and affordable housing. We meet immediate needs with food, clothing, and healthcare, and offer the tools one needs to live with hope, dignity, and greater independence.
SOME (So Others Might Eat) is seeking a Care team member (Therapist or Clinician) to work as needed to support the residents of Jordan House & Mary Claire House
Compensation: Pay Range: $35.00 � $42.00 per hour (based on experience and qualifications)
Position Description: SOME, Inc. is seeking a compassionate and skilled Care Coordinator to join the Jordan and Mary Claire House team on an as-needed basis. The Care Coordinator plays a vital role in supporting residents through individualized case management and service coordination to promote stability, self-sufficiency, and overall well-being.
This position works collaboratively with program staff and community partners to ensure each resident receives high-quality, trauma-informed care tailored to their unique needs.
Key Responsibilities

Conduct comprehensive assessments to identify residents� needs, goals, and barriers to housing stability.

Develop, implement, and monitor individualized service plans in collaboration with residents.

Provide referrals and linkage to community resources, including healthcare, mental health, employment, and social services.

Maintain timely and accurate case notes and documentation in compliance with agency standards.
-
Facilitate life skills coaching, crisis intervention, and advocacy on behalf of residents.
-
Participate in case conferences, staff meetings, and ongoing training to enhance service delivery.

Support residents� transitions toward independent living and sustained self-sufficiency.
Required Qualifications:

Bachelor�s degree in Social Work, Human Services, Psychology, or a related field preferred.

Minimum of one (1) year of experience providing case management or direct services to vulnerable populations.

Knowledge of trauma-informed care, motivational interviewing, and harm reduction principles.

Strong organizational, communication, and problem-solving skills.
-
Ability to work independently while maintaining collaboration with a multidisciplinary team.
Knowledge, Skills, and Abilities:
Knowledge of mental health issues and substane use disorders
Organized with an attention to detail
Ability to communicate with diverse populations
Time management skills
Ability to multi-task, despite competing priorities
Demonstrates good judgement to assist with client issues
Conflict resolution skills
Ability to set professional boundaries
Mission-oriented
Excellent verbal communication skills
Customer service skills to build a rapport with clients
Preferred: LICSW or LGSW, LGPC, LPC in Washington, DC
Reports to: Program Director
