An overview of this position entails…This position is responsible for providing nursing services and case management to residents in accordance with their scope of practice under their Registered Nurse (RN) license or practice under their Licensed Practical Nurse (LPN) license.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Demonstrate ethical and professional behavior that align with the values of the Hilltop’s Life Adjustment Program (LAP), including Service 1, how we treat the residents, and Service 2, how we treat each other. Service 1 and Service 2 is demonstrated by setting boundaries, being predictable, consistent, kind, and transparent.
2. Conducts oneself as a leader by taking ownership of the program, quality of care, commitment to the betterment and growth of BI Services by offering solutions, developing processes, promoting communication with others, supporting communication from staff to management, prioritizing tasks, representing your area at leadership meetings, and actively participating in problem resolution.
3. Provide nursing care to residents in the Life Adjustment Program per the Nurse Practice Act, physician orders, and individual program rules and regulations.
4. Conduct initial Nursing Assessments, including physical, psychological, psychosocial, and environmental aspects on each new resident, as residents enter the Program, per regulations.
5. Coordinate resident care with physicians, families, members of interdisciplinary team, and other involved agencies.
6. Document all care provisions and concerns in an accurate and timely manner.
7. Provide education and instruction to residents and their caregivers, working within the guidelines of licensure, certification and regulatory agencies to promote physical and emotional health in their home environments.
8. Provide in-service education to staff on various topics as needs arise.
9. Update residents’ families or caregivers on the residents’ progress, equipment needs, and health needs.
10. Assume responsibility for the delegation of nursing tasks to other health care workers and for the supervision of delegated tasks being conducted by other health care workers.
11. Collaborate and assist with other programs in the organization to provide support to residents and staff as needed.
12. Participate in the on-call duty rotation.

EDUCATION & EXPERIENCE
Current Licensure in good standing as a Registered Nurse (RN) in Colorado or Current Licensure as a Licensed Practical Nurse in Colorado, and 2 years of related work experience required.

- CPR Certificate/First Aid Certificate (required)
- De-Escalation Training (preferred)

- The starting wage range for this role is The LPN starting wage range is $27.00-$28.35 per hour. The RN starting wage is $35.00-$36.75 per hour.
