 The Licensed Practical Nurse (LPN) provides high-quality, patient-centered nursing care to residents in a skilled nursing facility (SNF). Under the supervision of a Registered Nurse (RN), you will assist in delivering and managing care for patients with various medical conditions, including those recovering from surgery, managing chronic illnesses, or addressing other health-related concerns. Your role is vital in ensuring residents receive compassionate, efficient, and effective care within the facility.

Position Details

• Status: Full-Time, Part-Time, or PRN
• Shifts: AM, PM, and 12-hour shifts available

Responsibilities

• Provide direct nursing care to residents in accordance with established policies and procedures.
• Administer medications and treatments accurately and safely.
• Monitor residents’ conditions and report any changes to the RN or physician.
• Collaborate with interdisciplinary team members to ensure quality, patient-centered care.
• Maintain accurate documentation and promote a positive, caring environment.

Requirements

• Current and valid LPN License (Ohio) in good standing (required)
• Minimum of 1 year of nursing experience (required)
• Strong clinical skills with attention to detail and patient safety
• Compassionate, dependable, and team-oriented
