Responsibilities

This position is responsible for the data entry, electronic medical record documentation and assistance with coordination of medication delivery, compliance and adherence. This includes clerical, technical, administrative, analytical and interpersonal support for the team exercising discretion, sound judgment and innovative thinking.

Accountabilities

1. Receives prescriptions and ensures proper enrollment to the designated specialty pharmacy.2. Closely follows the intake process with the designated specialty pharmacy and communicates any information accordingly to complete Rx dispense.3. Assists in co-pay and assistance programs as needed.4. Follows the intake and dispense process and ensures timely delivery of medication.5. Coordinates patient follow up visits after receiving medication.6. Communicates with patients via phone call and/or in person and assists in identifying patients with barriers, including but not limited to financial toxicity, transportation, housing and psychological.7. Follows up with patients regarding their progress and feedback regarding their treatment and care.8. Performs electronic medical record review, data entry and tracking.9. Works collaboratively with staff and physicians to ensure optimal health outcomes for patients.10. Performs clerical and administrative tasks (i.e. making photocopies, sending faxes, operating office equipment, data entry, etc.).11. Performs other duties as assigned.

Qualifications

Education• High School Diploma, General Education Development (GED) or Completion of a CMC Approved Individualized Education Plan (IEP) Certificate required

Licenses and Certifications• RXTECH - State of California Pharmacy Technician License required
