The Sr. Manager, Care Management is responsible for providing administrative and clinical leadership of the care management and quality teams, vendor relationships and the strategic development of clinical initiatives to achieve outcomes.  Ability to function in a matrixed environment with both internal and external stakeholders and support a high collaboration environment with our business partner.  

Location:

This is a hybrid position where the individual will split their time between working in our office in Miamisburg, OH and remotely and must be based in the Miamisburg , OH area.

Essential Job Functions:

• Provide oversight, direction, leadership and mentoring to supervisors and staff functioning in case management, care coordination, quality and population health.

• Apply analytic skills to develop clinical and operational initiatives to drive outcomes and improve organizational performance.

• Set performance goals and standards of care in collaboration with market leaders and medical directors.

• Collaborate with and oversee clinical vendor operations to optimize patient care.

• Demonstrate independent thinking, problem-solving and judgement.

• Engage with and influence physician partners to identify population health opportunities and implement mutually agreeable solutions.

• Establish implements and maintains department policies, procedures and processes to ensure compliance.

• Complies with organization policies and procedures.

• Adhere to national or agilon clinical guidelines.

• Passionate about value-based care, the importance of its role in our current healthcare system, and the necessity to achieve these goals through partnership.

• Proven track record managing clinical programs, particularly around value-based initiatives

• Strong operational and process management skills.

• Ability to concurrently drive multiple high priority workstreams.

Required Qualifications:

• Registered Nurse (RN) with Active state License – BSN required

• 6+ years’ experience nursing with 4 years leadership experience preferred

• Demonstrated ability to review, interpret and analyze data and understand business implications of clinical operations and plans

• General knowledge of health plan and physician practice operations and government programs

• Strong computer and Microsoft Office skills

• Must demonstrate strong verbal and written communication skills.

• Be able to work flexible hours as needed

• Demonstrate ability to work as part of the team
