Description

MDS Coordinator - LPN / RN Overview

The MDS Nurse (LPN/RN) oversees and facilitates the completion of Resident assessments in compliance with state and federal regulations. As MDS Nurse, you will coordinate, manage, monitor, and maintain accurate coding of the current MDS and care plan documentation.

  • Acts as a facilitator in developing a comprehensive assessment and care plan for Residents
  • Ensures timely scheduling and completion of the initial care plan and quarterly/annual review
  • Promotes optimal communication among program staff members, facility staff, referral sources, physicians, and residents
  • Participates in quality improvement activities to reduce cost and increase efficiency through improved systems and processes
  • Assess prospective residents in conjunction with other team members
  • Participates in patient/family care conferences; Care meetings as required and requested

Qualifications

  • Knowledge of regulatory standards and compliance requirements.
  • Freedom from illegal use of drugs.
  • Freedom from use of and effects of use of drugs and alcohol in the workplace.
  • Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for employment in this position.
  • Working knowledge of personal computer and software applications used in job functions (word processing, graphics, databases, spreadsheets, etc.).
  • Strong organizational and analytical skills; oral and written communication skills.
  • Demonstrates ability in independent functioning and strong leadership.
  • Working knowledge of MDS rules and regulations. 
  • Graduate of an accredited school of nursing.
  • Valid license as a Registered or Licensed Practical Nurse in Michigan.

Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and policy procedure manuals. Ability to effectively present information and respond to questions from groups of executive, managers, clients, customers, and the public.

Reasoning Ability
Ability to define and solve problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in statistical or diagram form and deal with several abstract and concrete variables.

Care and Services:

  • Assists the Resident Care Coordinator to fulfill responsibilities, as directed.
  • Serves as a resource person for resident care planning activities.
  • Completes MMQ and turnaround documents in a timely manner and communicate as needed with appropriate state departments.
  • Ensures that the quality and appropriateness of resident care meets or exceeds company and industry standards and ensures that all nursing services are in compliance with state, federal, legal, regulatory, accreditation, and reimbursement guidelines.
  • Supports program development activities.
  • Attends and participates in department/facility meetings, as required.
  • Accesses continuing education opportunities appropriate to responsibilities of the job.
  • Promotes optima communication among program staff members, facility staff, referral sources, physicians, and residents.
  • Appropriately accesses the resources of the company’s regional offices for consultation and program development support. Seeks out external resources through conferences, workshops, etc. as necessary. Routinely shares personal knowledge with staff, other managers, and facility personnel.
  • Demonstrates flexibility in adjusting to change, resident needs, and customer expectations; meets company standards relating to deadlines and attendance.
  • Established and maintains exemplary standards for department appearance and staff presentment; ensures compliance with company and facility policies and procedures.
  • Adheres to all safety and risk management protocols and programs.
  • Participates in direct and indirect resident care activities as warranted.
  • Participates in quality improvement activities, which reduce cost and increase efficiency through improved systems and processes.
  • Exhibits courtesy, compassion, and respect to residents, families, visitors, physicians, administrators, and coworkers.
  • Performs other duties or functions as directed.

Physical Demands

While performing the duties of this job, the employee is frequently required to stand and walk. The employee is occasionally required to sit; use hands and fingers to feel, handle, and touch; reach with arms and hands; and talk or hear. Occasional physical effort with medium to heavy objects may be required. Transferring residents weighing between 100 to 250 pounds is occasionally required. Specific vision abilities required by this job include close vision, distance vision, and peripheral vision.
