RESPONSIBILITIES
Including, but not limited to:

Reports to the Director of Nursing as Assistant Director of Nursing

Assures all necessary mandatory education related to fire safety, infection control, resident rights, hazard communication, safety issues, dementia training and policy and procedure are up to date.

Schedules mandatory in-services on a rotating basis.

Assists in developing, implementing and conducting relevant in-service training programs and staff-based competencies.
-
Orients/mentors new staff.
-
Provides staff education & development and on-going clinical competencies.
-
Assist with tracking & administration of staff and resident immunizations.
-
Share 'on-call' responsibility.

Assists Director of Nursing as needed.
-
Monitor resident progress and respond to changes in condition.

Collaborate with multidisciplinary teams to provide comprehensive care.

Educate residents and their families regarding health conditions, treatment plans, and discharge.

Maintain accurate resident records and documentation in compliance with state and federal regulations, while ensuring compliance with HIPAA regulations.

Participate in case management activities to ensure continuity of care.

Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc.
SKILLS
-
Registered or Licensed Nurse in the state
-
2 years or more of nursing experience
-
Leadership experience
-
Medication administration
-
EMR: PCC (Point Click Care)
-
Proficiency in memory care techniques and protocols.

Knowledge of MDS (Minimum Data Set) assessments for long-term care facilities.

Familiarity with operations in Skilled Nursing, Sub-Acute, and Dementia Units.

Strong background in case management practices.
-
Ability to work effectively in a medical office environment.
-
Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.

Strong organizational skills to manage multiple tasks efficiently.
