π Community-Based Role with Regional Travel
Lutheran Social Services of Wisconsin & Upper Michigan is seeking a dedicated and compassionate HUD Service Coordinator to support residents across multiple housing sites in Baldwin and surrounding areas.
This role is designed to promote independence and self-sufficiency for individuals residing in HUD-supported properties. Youβll connect tenants with vital community services, reduce early admissions to assisted living, and help residents maintain their independence for as long as possible. π
π§ What Youβll Do
- π§Ύ Provide informal case management, intake, and referrals
- π§ββοΈ Assess health, psychological, and social needs of tenants
- π§© Develop and monitor individualized service plans
- π§βπ€βπ§ Build relationships with local service providers and maintain a resource directory
- π£οΈ Educate tenants on available services, rights, and application processes
- π§βπ« Present workshops and training sessions on topics of interest
- π€ Foster informal support networks among residents, families, and volunteers
- π§βπΌ Collaborate with property staff and educate on aging-in-place strategies
- π Maintain accurate documentation and case records

- Ann Crider (Baldwin, WI) β 16 hrs/week
- Northern Lights (Hayward, WI) β Quarterly, 2 hrs/visit
- Shell Lake Shores (Shell Lake, WI) β Quarterly, 2 hrs/visit
- Tiffany Creek (Glenwood City, WI) β Quarterly, 2 hrs/visit
- Croft Place (New Richmond, WI) β Quarterly, 2 hrs/visit
- Prairieview Apartments (Star Prairie, WI) β Quarterly, 2 hrs/visit
- Oakview Apartments (Woodville, WI) β 6 visits/year, 2 hrs/visit
- The Eddie (Eau Claire, WI) β Monthly for first 3 months, then quarterly (2β4 hrs/visit)

π Qualifications
- π Bachelorβs degree in Social Work, Psychology, Sociology, or related field OR equivalent combination of education and/or experience is required with a minimum of 3 years working with older adults
- π§ Knowledge of supportive services and resources for older adults and/or individuals with disabilities
- π¬ Strong verbal and written communication skills
- π» Comfortable with basic computer systems and documentation tools
- π Valid driverβs license, reliable transportation, and auto insurance (MVR check required)
π Work Environment
- Office-based with regular travel to client homes and community locations
- Moderate noise level; occasional exposure to household allergens and outdoor conditions
- Physical activity including bending, stooping, and stair climbing may be required
