The Care Connector is responsible for supporting pregnant and postpartum members.  The Care Connector will facilitate and coordinate care for maternity members and will serve as a single point of contact in the Bright Start Maternity Program.  The Care Connector assesses maternity members to determine care coordination needs, identifies problems/barriers, and addresses member issues/concerns.  Implements the appropriate level of intervention based upon the member’s needs, schedules follow up calls as necessary and makes appropriate referrals.  Coordinates care and services with member, providers, and Community Based Organizations as appropriate.

Work Arrangement

Remote, must reside in Pennsylvania to be considered
Education/Experience:

Must be bilingual in Spanish
High School/GED
Minimum of 2 years previous work experience, preferably in a health care setting
Managed care experience preferred. 
Medical terminology/documentation experience is a plus
Prior telephonic customer service experience desired
 
Skills & Abilities

Excellent interpersonal and verbal/telephonic communication skills and the ability to write clear and concise documentation.
Attention to detail and confidentiality with strong organizational skills
Proficient PC skills in a Windows based environment including Outlook and Teams
