Job Title: Psychiatric Mental Health Nurse Practitioner, FACT
Job Summary:
The purpose of Forensic Assertive Community Treatment (FACT) is to deliver comprehensive and effective services to individuals (age 18 and up) who are diagnosed with serious mental illness (SMI) and whose needs have not been well met by more traditional service delivery approaches. These individuals also suffer from co-occurring substance use disorders and have histories of criminal legal system involvement. Assertive Community Treatment (ACT) is an evidence-based practice that provides an integrated set of other evidence-based treatment, rehabilitation, case management, and support services delivered by a mobile, multi-disciplinary mental health treatment team to support recipient recovery through a highly individualized approach that provides recipients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects.
The Psychiatric Mental Health Nurse Practitioner, along with the team leader, has overall clinical responsibility for the delivery of high-quality clinical services to the FACT Team consumers. The Psychiatric Nurse Mental Health Practitioner provides psychiatric assessment and evaluation, psychopharmacological treatment, and coordination of consumers’ health care needs.
Salary: The salary range for this role is $170,000 per year.
Shift Hours: The Psychiatric Nurse Practitioner is full-time position working Monday – Friday from 9:00am to 5:00pm.
Location Address: The position may be based fully at 151 Lawrence Street, Brooklyn, N.Y. 11201 Our office is easily accessible by public transportation.
Workplace Flexibility: Flexible (for roles that have days that can be partially remote, e.g., being able to draft notes at home after going to the office/community in the morning) to be discussed with your manager.
What You Will Be Doing:
- Conducts initial comprehensive psychiatric evaluations and monthly psychiatric assessments in the community.
- Prescribes medications through e-prescribing function of agency’s electronic health record, educates consumers about their medications and side effects, and monitors consumers’ response to prescribed medications.
- Coordinates consumers’ medication management needs with the FACT nursing staff, including ordering blood work and/or toxicology testing when indicated, managing prior authorizations for medications, administering injectable medications when needed.
- Completes and signs assessments, progress notes, incident reports, and other required documentations in accordance with the New York State Office of Mental Health (OMH) regulations, the New York City Department of Health and Mental Hygiene (DOHMH) guidelines, and Medicaid billing standards and regulations.
- Coordinates care with family and other community service providers or collaterals including, but not limited to: hospitals, housing providers, shelter staff, criminal justice personnel, Assisted Outpatient Treatment (AOT), and primary health care providers.
- Participates in team meetings and clinical meetings to collaborate and discuss clinical issues and clients’ needs with other team members.
- Uses motivational interviewing, developmentally and culturally appropriate, recovery oriented, and trauma-informed approaches when delivering services
- Helps maintain a safe and clean environment, ensuring proper use and disposal of medical supplies/equipment as required, adheres to infection control protocols, and notifies their supervisor immediately of any potential safety hazard identified in the office/client-care environment.
- Any other duties are required by ACT Leadership and senior executive staff.
What We Are Looking For:
- Currently licensed as a Psychiatric Nurse Practitioner by the NYS Education Department.
- Current board certification by The American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
- Extensive experience working with adults with serious mental illness, co-occurring substance abuse, who may also be justice-involved, homeless, with limited social support.
- Excellent written and communication skills.
- Bilingual Spanish speaking preferred.
