Job Summary

Perform home health aide services by following a written plan of care. Work is performed under the supervision of Clinical Operations. HHA will provide personal care and related activities of daily living services for patients in their homes, skilled nursing facilities and assisted living facilities. Reports and documentation will be prepared and regulatory and agency guidelines will be followed. Key Responsibilities • Reports are completed immediately in regards to abuse and neglect, according to internal and external guidelines. • Day sheets and activity sheets are documented accurately and within established time frames. • Care is provided in accordance with confidentiality standards, e.g. HIPPA. • Care is provided by adhering to plan of care, which may include end of life care and the bereavement process. • Care is provided that encourages patient’s optimal level of independence. • Care is provided using body mechanics that follow Agency guidelines. Education/Degree Requirements Required: High school diploma or GED Licensure, Certifications or Registration Required: Massachusetts Home Health Aide Certification or MA CNA certification required Experience • 1 years of home health aide experience preferred • Computer experience or demonstrated aptitude to learn • Must be able to communicate verbally and in writing to meet communication and documentation requirements • Strong interpersonal and customer service skills required to interact with a diverse population • Ability to adapt to changing situations required • Must be organized and be able to independently and reliably follow specific instructions Physical Environment Primarily will work in patients’ homes. Potential exposure to blood, blood products, body fluids and chemical hazards.

Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only preferred Certified Home Health Aide - Data Conversion - Various Issuers preferred

Experience
Clinical/Hospital/Managed Care Office Experience 1-2 years preferred or Patient Care Experience 0-1 year preferred

Knowledge, Skills and Abilities
- Knowledgeable in patient care practices including personal care, homemaking and basic health care services.
- Ability to work with limited supervision.
- Ability to follow written and verbal instructions.
- Have good personal hygiene.
- Able to do light housekeeping.
- Good communication skills both written and oral.
