Job Summary:

Provides psychosocial assessment, counseling, crisis, intervention, and information/referral services on a consultation basis and to improve patient health outcomes. Works collaboratively with the management team to ensure the efficient utilization of resources for positive patient outcomes.  Provides direct services to patients, families, and group.

Qualifications:

Education:   Master’s Degree Social Work required (MSW).  Licensed Social Worker (LSW) or Licensed Clinical Social Worker(LCSW) preferred.  Bilingual English/Spanish.         

Experience:  One year experience working as a social worker or applicable mental health experience required.  Two years’ experience working as a social worker in clinical / ambulatory care setting. A year of experience working in a Federally Qualified Health Center.
