Job Description
Overview of Role / Essential Job Functions
The Ward Clerk position exists to provide clerical support primarily to the nursing department. Ensure that the unit stock supplies, and unit communications are organized in a way to allow a high degree of supportive care to be provided to the residents and maintains the medical records in accordance with State and Federal regulations as well as professional standards of practice and facility policies and procedures to ensure complete, timely, and accurate medical records.

Resident Rights
- Knows and respects patient rights.
- Ensures all protected health information is kept confidential.
- Reports all complaints made by patients to supervisor.
- Reports all allegations of resident abuse, neglect and/or misappropriation of patient properly.
Safety and Sanitation
- Follows established safety policies and procedures.
- Observes safety needs of patient as indicated in care plan.
- Wears and/or uses safety equipment and supplies when indicated and properly trained to use.
- Demonstrates job-specific knowledge of fire and disaster preparedness during drills or actual situations.
Staff Development
- Attends and participates in scheduled in-service training, educational classes and meetings, including safety committee.
- Participates in QAA process.
Qualifications
Responsibilities
- Maintain resident medical charts to insure they are up to date by: a) filing medical information e.g. lab reports, Dr. progress notes etc., in appropriate chart, b) thin resident charts.
- Assemble chart packets for new admissions.
- Assist nurse during admission of a new resident with inventories list, marking glasses, teeth, etc., setting up chart and entering computer, etc.
- Maintain unit charge system by a) placing charge labels on incoming supplies, b) monitoring charge cards in notebook, c) do resident billing monthly and submit to Business Office.
- Monitor unit supplies and stock as needed.
- Monitor unit supplies for adequacy of stock and order supplies monthly.
- Assist with bi-annual of linen and annual inventory of supplies.
- Monitor and maintain supplies received from A-1 Home Healthcare by a) distributing supplies to appropriate areas, b) monitor supplies, order diabetic, inventory urological supplies for ADON.
- Assist nurse in verifying accuracy of newly printed medication sheets for succeeding month.
- Maintain and distribute department documentation sheets such as: a) CNA assignment sheets, b) incontinence product check-off sheets, c) bedrail documentation sheets (d) snack and meal documentation sheets, etc.
- Assist with ordering resident medications etc. from pharmacies by fax daily.
- Send phone orders, death records, etc. to physicians for completion weekly.
- Accompany residents on medical appointments, if the family is unable to make other arrangements.
- Review nurses’ charting on computer to keep current.
- Adhere to facility dress code: practice good personal cleanliness and grooming.
- Notify the facility in advance of absence or tardiness.
- Attend and participate in scheduled training, educational and orientation programs. Provide review in-services to staff monthly.
- Carry out emergency procedures according to facility policy and procedures (e.g. fire, tornado etc.).
- Follow all safety precautions according to facility policy.
- Follow established policy and procedure regarding exposure to blood/body fluids.
- Maintain confidentiality.
- Keep in-service records current – midyear send out form for number attended. Set up light watchers for in-services.
- Update CNA work assignments and assign halls for CNA's.
- Monthly – check paper supplies on ends and run copies as necessary to restock for nurses.
- Sort and distribute donated clothing.
- Fax necessary reports from nurses to Dr. Office daily. Pick up returned faxes from the Office and distribute them to appropriate nurse’s station.
- Fluid monitoring –Write up sheets per Care plan list for aides and dietary supervisor. Distribute to ends and dietary to keep track of 3 days of fluid intake. Pick up at end of 3 days and figure total fluid intake. Pain assessments – Write up sheets for nurses to do chart pain assessments for Assessment nurses per Care plan list. Distribute to ends. Put in treatment books. Log sheets – Run copies from master set of log sheets for aides to chart on - distribute.
- Accompany residents on dialysis to RMC per GNH van policy of driver and one other. Pick up lunches (if needed) and any papers from Nurses.
- All employees are expected to support the Eden program adopted by the Good Neighbor Home. This program is for the residents’ enjoyment and there may be occasional tasks required of the employees to maintain orderliness and cleanliness.
- Additional duties will be assigned by the supervisor as needed.

Education/ Experience/ Physical Demands
Education and Experience
- High School diploma or equivalency.
- Certified as a Nursing Assistant according to current state requirements.
- Proficient in the operation of a computer system.
- Possesses basic clerical skills necessary for the nursing staff.
- Possesses basic knowledge of medical terminology and/or Long-Term Care procedures.
- Able to work efficiently throughout the workday.
Physical Demands
- Primarily large in nature with exerting up to 60 pounds of force to lift, carry, push, pull or otherwise move objects, including the human body; climbing, balancing, stooping, kneeling, crouching, reaching, handling, feeling, talking, hearing, tasting, smelling, near acuity, far acuity, depth perception, accommodation, color vision.
