I.    JOB SUMMARY
Responsible for Home and community-based services and In-home support services intake process to ensure that all state and federal regulations are met, and company policies are upheld. Responsible for establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns.  
II.    DUTIES & RESPONSIBILITIES
1.    Coordinates with case managers to ensure the delivery of safe effective and appropriate home care services for the Home and community-based services and In Home Support Services program (s). 
2.     Completes intakes for PCP/Homemaking operations including providing direct oversight of the establishment and implementation of intake policies.
3.    Ensures compliance with all state and federal referral/intake regulatory requirements.
4.    Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
5.    Establishes and maintains positive working relationships with current and potential referral sources.
6.    Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
7.    Meets with field staff on a regular basis to provide guidance and updates.
8.    Provides training to field staff regarding regulations, care plans and general tasks in clients’ homes.
9.    Performs supervisory of care providers
10.    Audits HCBS charts for effectiveness of care coordination and supervisory.
11.    Communicates to clients, case managers, scheduler, and providers of any changes to schedule or care plan.
12.    Performs non-skilled training and sign offs to employees
13.    Back up coverage to C.N.A., IHSS, PCP and Homemaking visits
14.    Assists with recruiting, selecting, orienting, and supervising
15.    Builds and monitors community and customer perceptions of PASCO/SW Home Health as a high-quality provider of services.
16.    Gathers, collates, and reports referral statistics including key customer referral trends.
17.    Provides leadership in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs.    
18.    Maintains comprehensive working knowledge of contractual relationships and ensures that patients are admitted according to contract provisions.
19.    Participates in quality assessment performance improvement teams and activities. Assists with reports and participates in QAPI meetings 
20.    Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by PASCO/SW Home Health.
21.    Participates in the On-Call Rotation 
22.    Assist with scheduling functions

III.    JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
1.    Certified Nursing assistant Must have successfully completed a training and competency evaluation program that is approved by the state as meeting all requirements and listed in good standing in the state nurse registry.
2.    Experience with client coordination, customer service, and case manage ment
3.    Organization skills to maintain accurate records for multiple clients
4.    Strong oral and written communication skills to convey health information in and easy-to-understand manner.
5.    Experience or knowledge of Home and community-based services (HCBS)
6.    Experience with Medicaid HCBS Waiver and Home Healthcare state regulations
7.    Ability to multi-task, prioritize, and manage time effectively.
8.    Demonstrates good communications, negotiation, and public relations skills.
9.    Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities 
10.    Extraordinary attention to detail
11.    Possess strong computer skills including Microsoft office suite, outlook, and Electronic Medical Records
12.    Commute to multiple client homes throughout the day to perform scheduled supervisory.
13.    A valid driver's license and a clean record are essential.
14.    Strong knowledge of traffic laws and safe driving practices

Salary - $18.00-$23.00
