KEY FUNCTION:
Deliver client-centred nursing services in collaboration with registered nursing staff and allied health professionals.

 

REPORTING:
Under the general direction of the Manager, Outpatient Clinic or the Manager, Systemic Treatment.

 

DUTIES:
1. Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards as outlined by the College of Nurses of Ontario and Division of Oncology Nursing.
2. Provide high quality nursing care through management of the patient care continuum, education, and advanced nursing practice.
3. Organize daily departmental work flow, conduct patient assessments, counsel patients and families, and provide care and treatment.
4. Participate in the multidisciplinary management of oncology patients in the ambulatory setting.
5. Address physical, psychological, social, spiritual and educational needs of patients and their families.
6. Maintain and coordinate communication between patient, family, staff and physician as evidenced within medical record, client, and health care team feedback.
7. Document pertinent observation and intervention on appropriate records.
8. Perform all nursing care duties within the RPN scope of practice.
9. Recognize changes in patient condition, assess, and consult appropriately with the RN or other health care team members.
10. Prepare patients for treatment as required.
11. Work in consultation with the health care team to meet care needs of more complex patients.
12. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
13. Participate in hospital training as required.
14. Assist with improvement initiatives as required.
15. Perform other duties as required.

 

QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a two (2) year diploma in Practical Nursing from an approved School of Nursing with a current certificate of competence with the College of Nurses of Ontario.
2. Current Basic Cardiac Life Support (BCLS) is required.
3. Certification in the management of Peripheral Intravenous and Central Vascular Access Devices is required.
4. Successful completion of de Souza Foundations in Oncology Nursing Practice or Cancer Care Basics is preferred.
5. Current certification with Canadian Vascular Access Association (CVAA) is preferred.
6. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE:
1. Minimum of two (2) years experience, within the last five (5) years, working as a nurse in oncology within full scope of practice, including medication administration.
2. Experience in ambulatory care and tele-health is preferred.

 

KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated knowledge of anatomy and oncology terminology.
2. Demonstrated knowledge and understanding of Cancer Care Ontario (CCO).
3. Demonstrated ability in patient assessment, teaching, and counselling.
4. Demonstrated working knowledge of all equipment used in the delivery of patient care.
5. Demonstrated computer skills with advanced proficiency in Microsoft Office software (Word, Excel, and Outlook) and patient information systems (e.g. Mosaiq).
6. Demonstrated interpersonal and communication skills.
7. Demonstrated ability to prioritize tasks.
8. Demonstrated compliance with HSN’s privacy policy.
9. Demonstrated commitment to the safety of co-workers and patients.
