Zepf Center has been serving the Lucas County community for over 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff.
General Summary:
The registered nurse is responsible for direct nursing services for Assertive Community Treatment (ACT) clients, including all duties listed. The registered nurse is professionally accountable for carrying out the nursing process of problem identification, planning care, interventions, evaluation of client response, and revision of plan of care, as appropriate, in accordance with ACT fidelity requirements.
Essential Duties and Responsibilities:
- Through client health assessment and observations, continuously collects and evaluates data that is comprehensive, accurate, and systematic.
- As part of the ACT treatment team, contributes to the development of an individualized treatment plan delineating specific actions and assumes responsibility for interventions.
- Implements nursing actions that promote, maintain, or restore physical and mental health, prevent illness and effect rehabilitation.
- Performs general health assessment, monitors health status and takes appropriate action to restore and maintain health within the scope of licensure including coordination with pharmacy and other providers regarding medication changes and compliance.
- Participates in daily ACT team meetings for planning, prioritizing and implementing daily tasks and assignments.
- Evaluates client responses to interventions in order to revise treatment plan.
- Provides, structures, and maintains a therapeutic in collaboration with the client and coworkers.
- Provides service in most appropriate setting based on client needs including regularly conducting field-based work.
- Works effectively and collaboratively within the multidisciplinary team including daily staff meetings and providing psychoeducation groups as needed.
- Participates in the management of emergency, provides assistance to other staff in case of emergencies and takes appropriate and positive action to control clients who are dangerous to themselves and/or others.
- Accurately documents service provision and billing information to assure reimbursement and compliance with fidelity requirements.
- Fulfills all administrative tasks and requirements.
- Health Officer designation required or obtained within 6 months of hire to support crisis interventions for ACT clients.
- Assumes responsibility for meeting the needs of each and every client they come into contact with.
- Provide on-call coverage during evening and weekend hours as required of members of the ACT team.
- Performs other duties and responsibilities as assigned.

- Knowledge and ability to administer and dispense medication in accordance with physicians’ orders.

- Ability to obtain and maintain licensure and provide documentation for personnel records.

Requirements
Position Qualifications:
- Must have a current Ohio RN license in good standing.
- At least 2 years or more of experience working with severe and persistent mentally ill population or other related nursing experience.
- Must have a valid driver's license.
- CPR and First Aid trained and crisis intervention (CPI) certified.
- Health Officer designation required or obtained within 6 months of hire.
- Must have the necessary skills and abilities required to comfortably and effectively work with patients, being responsive to their needs.
