A Day in Life as a Coordinated Entry Specialist:

As a Coordinated Entry Specialist, you’ll play a crucial role in facilitating and managing the coordinated entry process for individuals seeking housing assistance in the Belknap-Merrimack community of New Hampshire. As a key member of the Belknap-Merrimack Community Action Program, Inc., the Coordinated Entry Specialist will be responsible for assessing client needs, coordinating referrals, and connecting individuals experiencing homelessness with appropriate housing resources and support services. This position requires exceptional organizational skills, strong interpersonal abilities, and a deep commitment to helping individuals access stable housing.  solutions.

Your day will involve:

  • Conduct comprehensive assessments of clients seeking housing assistance in the Belknap-Merrimack community, including evaluating their housing needs, vulnerability, and eligibility for different housing programs.
  • Implement and manage the coordinated entry process within the Belknap-Merrimack community, ensuring a streamlined and efficient system for assessing, prioritizing, and referring clients.
  • Collaborate with local community partners, housing providers, and social service agencies within the Belknap-Merrimack area to gather and exchange relevant client information and coordinate referrals.
  • Maintain accurate and up-to-date client records and documentation in accordance with program guidelines and confidentiality protocols specific to the Belknap-Merrimack Community Action Program, Inc.
  • Provide support and guidance to clients throughout the housing search process, offering information about available resources, housing options, and eligibility requirements within the Belknap-Merrimack community.
  • Assist clients in completing necessary applications and forms for housing programs, ensuring accuracy and compliance with the specific requirements of the Belknap-Merrimack Community Action Program, Inc.
Requirements

Qualifications to Be a Coordinated Entry Specialist:

To excel in this role, you’ll need: 

  • Education: High School diploma required.
  • Experience: Two years of professional or personal experience with homelessness, mental illness, drug/alcohol abuse, or developmental disabilities.
  • Skills: Express courtesy and willingness to assist others. Maintain adequate knowledge of all CAPBM programs to make referrals to other CAPBM programs beneficial to the participant. Maintain professional boundaries with all current, past, and prospective clients, and maintain the confidentiality of clients and staff while adhering to the confidentiality of clients, co-workers, and internal business information.
  • Characteristics: Strong desire to assist others in helping themselves. Analytical and problem-solving techniques. Excellent organizational and communication skills. Ability to work independently with minimum supervision. Knowledge of local human service agencies and
  • Technical: Must successfully complete non-violent crisis prevention and intervention training. Successful completion of criminal record check.

Reliable transportation and a valid driver’s license are required, as some travel within the community may be necessary.
