Registered Nurse Navigator (RN)- Chronic/Traditional Care Mgmt bei BMH Memphis Hospital

Job Summary: The RN Nurse Navigator will oversee care for a designated population and ensure the secure flow of patients across the continuum of care, optimizing resource use, service delivery, and adherence to payer regulations. Offers evaluation of transitional requirements recognized following hospitalization, illness, and/or life circumstances. Engages with relevant personnel and insurance providers to facilitate seamless patient progression within the healthcare system. Performs other duties as assigned. 

Responsibilities:

• Manages a caseload of patients 
• Performance Improvement
• Acts as a liaison between the patient, physician, and other medical providers and insurance companies to ensure quality care for patients 
• Coordinates with appropriate staff and insurance payers to ensure smooth patient flow through the healthcare system
• Readmission Reduction Participation
• Payer communication and denial reduction
• Completes all assigned goals 

Requirements, Preferences, and Experience:

Education: 

Minimum:

• Licensed RN in the state of Mississippi
• Critical thinking

• Computer skills 

Experience:

• Preferred:
• Care coordination nursing experience 
• Case management experience 
• Minimum Required:
• BSN-RN or 3 years of nursing experience on a current unit or in Specialty Area

Licensure, Registration, Certification:  Minimum: 

• RN

Special Skills: Minimum: 

• Broad knowledge of community services, health concepts, and adult education
• Ability to be organized and prioritize workload
• Efficiencies in Microsoft Office Software 
• Ability to work well with teams
